3.Qualification Requirements For Bidder

Qualifications or Performance：1) The bidder shall have the independent legal person qualification, and register into a member at www.chinabidding.com. Domestic bidder’s bid documents shall provide copy of business license with bidder's official seal. 2) Copy of bidder’s business license or business registration certificate (The business scope must include the bidding products, except bidders from abroad). 3) If the bidder is production enterprises of the bidding products (3.0T MRI and DSA), shall provide the manufacturing license of medical equipment enterprise and the production scope includes the product; If the bidder is agents or authorized suppliers of the bidding products (3.0T MRI and DSA), shall provide the license copy of the original reference, and shall provide the business enterprise license of medical equipment, and the business scope includes the product (provide copy and stamped with the official seal). 4) Registration certificate and registration form of medical equipment issued by State Food and Drug Administration, the bidding products (3.0T MRI and DSA) to production for the period of validity of their registration certificate of medical equipment (the certificate should be within the validity period in the bid opening; if not within the period of validity it need to provide the certificate and certificate documents that the bidding products are produced in the period of validity of the certificate), and also registration certificate and registration form of imported medical equipment if it is imported equipment.
